TIER I

  • 2 oral exams* per year

  • BWs or Pano X-ray as provider deems necessary

  • 2 dental cleanings (or 2 periodontal maintenance) per year

  • All x-rays as provider deems necessary
  • All other services discounted by 20%

*Oral exam includes comprehensive oral (new patient) or periodic oral (existing patient) exam. This includes General Health Assessment, Dental Caries Check, Periodontal Evaluation, Oral Cancer Exam, Sleep Apnea Screening, Intraoral Pictures and Diagnostic Photos as needed.

Both TIER I and II include one Limited/Problem Focused Exam and Single x-ray per year

Limitations

  • This is not a dental insurance plan.
  • The annual membership fee is due in full upon registration.
  • Membership is effective on the date initial payment is received.
  • It is the responsibility of the member to take full advantage of their plan by scheduling all appropriate appointments within the I 2 month period. If the appointments are not scheduled or appointments not kept, the member is NOT entitled to a refund.
  • A 48 hour cancellation policy will be enforced. Two non-kept appointments will result in a Non-Refundable Voided Membership.
  • Annual Membership is eligible for renewal after 12 full months.
  • The Membership is NOT a dental insurance plan. It is EXCLUSIVE to Dr. Muna Mohammed‘s dental clinic. It is not transferable and cannot be used in conjunction with any other dental insurance, worker’s comp, medical insurance, discount plan, or other specials.
  • Membership fee may be reviewed and adjusted on an annual basis with notification.
  • Cannot be used for treatment which, in sole opinion of treating dentist, lies outside the realm of their capability.
