1. Agreement to these terms
These Membership Terms & Conditions (the "Terms") form a legal agreement between you ("Member," "you") and Snow Tree Dental, the dental practice of Dr. Muna Mohammed, located at 2400 Augusta Dr. Ste #175, Houston, TX 77057 ("Snow Tree Dental," "we," "us"). By enrolling in and paying for the Snow Tree Dental in-house membership plan (the "Plan"), you acknowledge that you have read, understood, and agreed to these Terms. If you do not agree, please do not enroll.
2. The Plan is not insurance
The Plan is not a dental insurance policy and is not a substitute for dental or medical insurance. It is a discount and pre-paid preventive-care membership offered exclusively by Snow Tree Dental. It does not meet, and is not intended to meet, any legal requirement to maintain health or dental insurance coverage. No portion of your membership fee is an insurance premium, and the Plan is not regulated as insurance.
3. Membership plans & included benefits
The Plan is offered in two annual tiers. Benefits are provided at Snow Tree Dental only and are performed as clinically appropriate in the judgment of the treating dentist.
Tier I — Essential
- Two (2) oral examinations per membership year
- Two (2) dental cleanings per membership year
- Two (2) preventive fluoride treatments per membership year
- Bitewing or panoramic X-rays as the provider deems necessary
Tier II — Complete
- Two (2) oral examinations per membership year
- Two (2) dental cleanings or two (2) periodontal maintenance visits per membership year
- Two (2) preventive fluoride treatments per membership year
- All X-rays as the provider deems necessary
- A twenty percent (20%) discount on all other services provided at Snow Tree Dental
Both tiers also include one (1) limited/problem-focused examination and one (1) single X-ray per membership year. An oral examination includes a general health assessment, dental caries check, periodontal evaluation, oral-cancer screening, sleep-apnea screening, and diagnostic photographs as needed. Included visits do not roll over and have no cash value.
4. Membership fees & payment
Current annual membership fees are:
- Tier I — $225 for one person; $325 for two persons
- Tier II — $400 for one person; $600 for two persons
The full annual fee is due at the time of enrollment. We accept Visa, Mastercard, Discover, CareCredit, personal check, and cash. Any treatment not included in your tier, and any applicable member discount, will be billed at the time of service. You are responsible for all charges for non-included treatment.
5. Membership term, effective date & renewal
Each membership runs for twelve (12) consecutive months (the "Membership Year"). Your membership becomes effective on the date we receive your initial payment. Membership is eligible for renewal after twelve (12) full months. Renewal is not automatic unless we tell you otherwise in writing at enrollment; to continue, you must renew and pay the then-current fee.
6. Using your benefits
To get the full value of your Plan, it is your responsibility to schedule and attend all included appointments within your Membership Year. Unused visits expire at the end of the Membership Year, are not refundable, carry no cash value, and do not transfer to a renewal term.
7. Appointments, cancellations & missed visits
We ask for at least forty-eight (48) hours' notice to cancel or reschedule an appointment. Appointments cancelled with less notice, or not attended, may be treated as a missed visit. Repeated missed or late-cancelled appointments (for example, two or more within a Membership Year) may result in termination of your membership without refund, in our reasonable discretion.
8. Refunds
Except where required by applicable law, membership fees are non-refundable, including if you do not use all included benefits, cancel, or your membership is terminated for missed appointments or other breach of these Terms.
9. Exclusive, non-transferable & non-combinable
Membership and its benefits are exclusive to Snow Tree Dental, are personal to the enrolled Member(s), and are not transferable or assignable. The Plan cannot be used or combined with any dental insurance, medical insurance, workers' compensation, government program, other discount plan, coupon, or promotional offer. Discounts apply only to services rendered while your membership is active and paid.
10. Exclusions & limitations
The Plan covers only care that Snow Tree Dental provides in our office. It does not cover treatment referred to or performed by any other provider, laboratory fees billed separately by third parties, or any treatment that is, in the sole professional judgment of the treating dentist, outside the scope of our practice or not clinically appropriate. Nothing in these Terms obligates us to provide care that we determine is not in your best clinical interest.
11. Fee changes & changes to these terms
Membership fees may be reviewed and adjusted, generally on an annual basis, with advance notice before your renewal. We may update these Terms from time to time; the version in effect on the date you enroll or renew governs your membership for that term. The current version is always available on this page.
12. No guarantee of outcomes
Dentistry is not an exact science. We do not guarantee any particular result, and enrollment in the Plan does not create any warranty of outcome. All care is provided in accordance with the professional judgment of the treating dentist and applicable standards of care.
13. Termination
You may cancel your participation at any time, subject to the non-refund provisions above. We may terminate a membership for non-payment, missed-appointment policy violations, abusive conduct, or breach of these Terms. On termination, all unused benefits are forfeited without refund.
14. Privacy
Information you provide in connection with your membership is handled in accordance with our privacy practices and applicable law, including HIPAA where it applies to your protected health information.
15. Governing law & disputes
These Terms are governed by the laws of the State of Texas, without regard to its conflict-of-laws rules. Any dispute arising out of or relating to the Plan or these Terms will be brought exclusively in the state or federal courts located in Harris County, Texas, and you consent to their jurisdiction.
16. Entire agreement & severability
These Terms, together with any enrollment form you sign, are the entire agreement between you and Snow Tree Dental regarding the Plan and supersede any prior understanding. If any provision is found unenforceable, the remaining provisions stay in full force and effect.
